Kindly provide the following details for appointment purposes:
- Name
- NRIC Number
- Contact Details
- Health Screening Date
- Purchase Order ID
Important Notes
- Please make your appointment at least 5 working days in advance.
- Any appointment made under a different name from the purchaser must be highlighted while arranging your appointment.
- Proof of purchase must be presented on the day of appointment and at the registration counter.Â
- Additional charges may apply should there be any additional test or procedure advised by the doctor.
- Validity period of the purchased product differs based on the respective product as stated in the terms and conditions of each individual product.Â
- Preparations and instructions for health screening will be advised prior to your appointment. You may also click here for some common instructions.
- If you would like to get a copy of your official receipt, kindly approach the cashier/payment counter on your health screening day at our health screening centre.
